Address at Diagnosis--Supplemental allows for additional address information such as the name of a place or facility (for example, a nursing home, apartment complex, or other mailing address) at the time of diagnosis. This data item is collected by SEER registries for identification and matching purposes; it is not submitted to NCI SEER.

Coding Instructions

1. Record the name of the place or facility where the patient resided when the tumor was diagnosed
   a. For example, the name of an apartment complex or a nursing home

2. If the patient has multiple tumors, the address may be different for different primaries

3. Record PO Box information in this data item. Do not use this data item to record the number and street address of the patient. Record number and street in Address at Diagnosis--Number and Street

4. Do not update this data item if the patient’s address changes

5. Leave blank if this data item is not needed

This field is automatically filled in with the data in Item 10070 (Current Street Address- Line 2) when the case is initially entered into CPDMS.net.  It provides space to record additional address information, such as the name of a nursing home, apartment complex, etc. This line will be used as an alternate address line for geocoding. If Address at Diagnosis-Line 1 cannot be geocoded (i.e. PO Box), then this line will be reviewed for a geocode. Do not update this item if the patient’s address changes. Leave this field blank if the additional address space is not needed.
